DSA (Digital Subtraction Angiography) is the most accurate imaging technique in evaluating the cerebral vascular system in finding vascular abnormalities in the brain, such as arterial stenosis, venous arterial malformations, and cerebral aneurysms.

DSA (Digital Subtraction Angiography) is an invasive examination using catheters, wires, contrast agents, and angiography machines. This procedure is usually performed to find anatomical abnormalities of cerebral blood vessels such as aneurysms, AVMs, and stenosis. DSA can also determine blood flow and vascular conditions such as vasospasm, vasculitis, and brain tumor vascularization.

What is the Purpose of DSA (Digital Subtraction Angiography)?

DSA (Digital Subtraction Angiography) according to its purpose is divided into two, namely:

1. Diagnostics

Detects blood vessel abnormalities, tumor vascularization, and more. Patients only need to prepare by fasting for four hours, checking Hb and leukocytes, kidney function and blood sugar. Patients with diabetes mellitus should stop taking the drug the day before DSA.

2. Therapeutic

Treatment of abnormal blood vessels by inserting drugs, devices or implants into the vessels. DSA is also used as a complementary therapy before undergoing surgery.

Why does someone need DSA (Digital Subtraction Angiography)?

 DSA (Digital Subtraction Angiography) is used to diagnose:

1. Abnormal conditions between arteries and veins

2. Vascular diseases, including obstructive vascular diseases caused by blockage or narrowing of the lumen (inner part) of arteries and veins

3. Brain aneurysms (especially intracranial aneurysms)

4. Blood vessels

In addition to diagnosing some of the above conditions, this DSA (Digital Subtraction Angiography) procedure can also be used for:

1. Assessing the vascular system of cancerous tumors.

2. Provide visual guidance for interventional procedures such as angioplasty (ballooning) and vascular stenting.

What are the procedures performed during DSA (Digital Subtraction Angiography)?

When a patient undergoes a DSA examination, it is possible that a patient with a diagnostic purpose will also have to undergo therapeutic measures.

This technique is more widely used in cases of aneurysms and strokes, as the use of drugs becomes more targeted. Before undergoing DSA, there are several pre-procedural evaluations that the patient must undergo.

The DSA technique is performed in a special room called a cath lab, with the procedure using a fine tube inserted through a blood vessel in the thigh crease. During the DSA process, the patient is only given local anesthesia so that the patient is conscious during the procedure. DSA is a procedure that only takes approximately 40 minutes.

DSA is also useful for evaluating cases of cerebral hemorrhage or so-called hemorrhagic stroke, where cerebral hemorrhage occurs spontaneously which is usually triggered by uncontrolled high blood pressure, fragile blood vessels, and cerebral hemorrhage can also occur without an impact on the head or due to abnormalities in the structure of the cerebral blood vessels themselves.

For room set-up or DSA examinations, it should be equipped with hemodynamic monitoring equipment. The patient's oxygen, heart rhythm and rate should always be monitored, and blood pressure measured regularly and gradually. The entire procedure should be performed under strict aseptic conditions, including technique, preparation and attire.

DSA is an examination that uses a fluoroscopic radiology angiography device. The angiography plane uses X-rays, in real time or continuously, to monitor the examined blood vessels after injecting contrast. So that the blood vessels are seen from the arterial phase, capillary phase, to the venous phase.

The side effects of DSA are the displacement of the vessel with the catheter, or tearing of the blood vessel. These side effects are much smaller compared to the procedures, which had to be taken before this technology was developed. Where patients had to undergo vital surgeries such as skull opening, which could result in infection.
